Registration Documents/Forms

In addition to completing your camper's profile on CampDoc, you must also have completed the following forms. Please note, many require a legal guardian's signature.

Medical Packet - includes 4 Sections

• The Medical Packet is composed of 4 sections.
• Doctors FREQUENTLY forget to check or sign off on sections within this packet. Please double check that your physician checked and signed off every section before leaving the office, even if your camper has minimal health issues.
• If you have any troubles, please contact us so we can help.

Section 1:
​Medical Form

The Medical Form must be completed within 12 months of attending their camp session. We also accept sports physicals. This must be signed by a doctor.

Section 2:
​Medication List 

The Medication List must be a list provided by and signed by the doctor and should include the following:

• Over-the-Counter Medications that we are permitted to administer such as tylenol, midol, miralax, allergy meds, etc. Double check that your doctor also lists the dosage & signs the list before leaving the office. We cannot administer any over-the-counter medications without a physician provided list or physician orders for each medication. 
• Prescription Medications that the camper will take while at camp. If your camper has frequent medication changes, that is okay. At your camper's physical, get the most current medication list. If it dosage or medication changes prior to camp, you or your physician can fax or email us the updated list that includes any changes prior to camp arrival.  We cannot administer medications without a physician provided list or physician orders, even if the prescription label is correct. *Note: you must also manually enter medications and medication changes in to CampDoc.. They must match what is written on the Medication List.

Section 3:
​Medical Protocols

The Medication Protocol Form must include all behavior and medical protocols (or safety plans) set in motion by a medical professional. This must be signed by a doctor. 

Section 4:
​Immunizations

Immunizations must be current unless there is a medical reason. Tetanus expires every 10 years. Please attach immunization record.

Consent Form

The Consent Form must be read and signed by both the camper and the camper's legal guardian. If the camper is their own guardian, the camper must sign it.

The Camper Rights & Responsibilities Form must be read and signed by both the camper(if able) and camper's legal guardian. If the camper is their own guardian, the camper should sign both lines. 

Campership Application (optional)

If your camper is in need of financial assistance beyond our Pick Your Price Tiered Pricing Model, first apply for your camper's session of choice, THEN send us the Campership Application. Before applying, be sure to read about our new Tiered Pricing Model.

• Our funds are limited and dependent on our generous donors.
• Please note that a copy of your most recent tax return, SSDI/SSI award letter, or proof of any other income listed on the application are required. Mail the completed form and required documentation to Easterseals Nebraska, Attn: Holly Windorski, 12565 W. Center Road Omaha, NE 68144 OR fax to 888-611-6396.
• Camperships will be awarded on a first come, first completed, first serve basis! Notification of Campership Awards will begin in March.
• If you don't qualify for a Campership, please consider our Pick Your Price Tiered Pricing Model or check out our "Alternative Funding Ideas" on the Cost & Payment Options page to help you supplement your camper's tuition.
